Date: April 2012-October 2012
Vendor Location: Pump Parts Fabricator in Nuevo Leon Mexico.
Review on CMTRs per ITP-GM-038 Rev 2, Item E7 and Drawing S542789 Rev B.
Items Inspected: Volute Casing Part Number R76U67592, Specified Material: ASTM A-487-CA6NM, Heat Number W8451-1 and Y2191-4.
Findings: No Reference on CMTR on Compliance to NACE MR 0175 Specification for reduced hardness measured in weld metal and heat affected zone including repair welds as specified on Referenced ITP Item E7.
Re-Inspection: Vendor has conducted the required Hardness Test and confirmed on Compliance to NACE MR 0175 specification, as well accordingly the CMTRs have been revised to reflect on it.
Audit on UT Examination per ITP-GM-038 Rev 2 Item E9, Vendor UT Procedure GM-039-02 Rev 2, ASME Section V Article 5/Article 23 and ASME Section VIII Div 1 Appendix 7.
Items Inspected: All Castings within supply scope from Vendor.
Findings: No Full Compliance to requirements per documents above was found for Calibration of Ultrasonic Device, Material of Calibration Block, Surface Temperature of Material being Examined, Calibration Reflectors, Straight and Angle Beam Scanning and Sensitivity of Examination.
Re-Inspection: Per Observations from Fraza Team, Vendor has worked to address properly on all referenced UT Examination aspects achieving compliance to Project Specifications.
Witness on Repairs by Welding application on Excavated Areas per ITP-GM-38 Rev 1 Item E8 and WPS-NG-5087 Rev C.
Item Inspected: Case Casting, Part Number M76B65503, Material: ASTM A-487-CA6NM, Heat Number E9436-4.
Findings: Operations of Weld Preparations and Cleaning being done by grinding and brushing with tools previously used on other type of materials with a serious risk to contaminate the surfaces being prepared.
Re-Inspection: Addressed by Vendor, repairs and cleanliness completed by grinding on all weld preparations using only New Austenitic Stainless Steel Tools.
Benefits to Our Customer: By using Fraza Inspection Team, Customer saved a lot of of time and money preventing excessive expenses per back charges for Extra-UT Examination and Reworks at field site per unacceptable issues on Omitted Tests, Not Compliance to Project NDE Requirements and Not Proper Weld Peparations, If Not Corrected/Repaired in timely manner at Vendor Shop.
