Item Inspected: Blower Skid Structure.

Findings: Approximately, ten areas showing Mudcracking Observed mainly on flange edges of beams under floor plates, areas between webs and flanges of beams and other ones on external gussets and external side of beam near lifting lugs.

Re-inspection: All Needed Repairs done by vendor in accordance with Customer Coating Specification BS-018-01 Rev 2, as confirmed by visual inspection. See Photos on Described Mudcracking Issue.

Item Inspected: Pump Tag BHM-60-T-0310-A/B/C.

Findings: Specified Paint Systems per Approved Painting Procedure BHM-040-R-050A Rev C are as follows: Casing and Cover, Paint System BHCS-22C- Surface preparation: Clean to SSPC-SP1-Primer: International Paints Intertherm 751 CSA(25-65 microns). Bearing Housing, Paint System BHCS-4A, Surface preparation to Sa3, Primer, Hempel Paints Hempadur 85671 or International Paints Inter Line 399(95-125 microns). Due to a lack of communication between Main Vendor and Manufacturer in Mexico, Wrongly, a Paint System per Paint Specification BQS-760033-0010 Rev 0, with Primer, International Interzinc 22(50-80 microns) has been applied by Mexican Manufacturer on Casing, Cover and Bearing Housing.

Re-Inspection: Per Customer Disposition, Casing, Cover and Bearing Housing have been Re-Blasted and Re-Painted in accordance with Paint Systems per Approved Painting Procedure BHM-040-R-050A Rev C referenced Above. See Photos before and After Re-work on Paint.

Visual Inspection on Surface Preparation per Vendor ITP 790033-030 Rev 2, Item 3.6 and Approved Painting Procedure BHM-040-R-050A Rev C.

Item Inspected: Pump Tag BHM-80-T-0620-A/B/C.

Findings: Per Section 7.8.2 from Approved Painting Procedure referenced above, Prior to blasting, rough welds and sharp edges shall be ground to a minimum radius of 3 mm to ensure proper coating application. However after Primer Coating Application, several áreas from casting and machined surfaces still were observed with Sharp edges Not ground to a minimum radius of 3 mm as specified.

 Findings: Suction/Vent Nozzles-Flanges Item 3 on both Pumps, Observed with flange facing finish(Roughness) seriously damaged, apparently the gasket seats finish was exposed to the action of Blasting during Surface Preparation:

Re-Inspection: A NCR-1023-01 has been open on this Issue and per Customer Disposition, Flanges damaged have been cutted off and new flanges have been installed and welded. As well a new Hydrostatic Test has been conducted for the Reworked Nozzle Header as per reviewed records as provided from Vendor. See Photos on Referenced Issue.

Findings: Stairways and Ladders observed just with hot dip galvanizing done, due to an incorrect interpretation of the project requirements by Vendor, the specified painting application with International Paints Inter Line 399(100-125 microns) over Galvanizing per Appendix "B" item 3.7 from referenced Coating Spec has not been done.

Re-Inspection: Per observation from Fraza Team, Vendor has completed on painting over galvanizing in accordance with specified Paint System per Coating Spec referenced above.  See Photos on described Issue as originally found.

Items Inspected: Exhaust Transition Item 35.

Findings: Primer Coating was being applied on outside(Out of Painting Booth) with the serious risk to contaminate the fresh coating just applied by the dust being blown by the wind. Fraza Team has requested to Vendor QC to avoid this Inadequate Practice.

Re-inspection: The referenced Equipment has been moved into the Painting Booth and Primer Coating completed there. Addressed by Vendor. See Photo.

Visual inspection on Painting per ITP-50921-01 Rev 2 Item 11 and Customer Paint Specification- QQT50920 Rev 2.

Items Inspected: Exhaust Elbow Duct Item 38.

Findings: An important number of locations on welds of external stiffeners and on channels attached to shell plates observed with Mudcracking on Primer Coating due to an excessive paint application. Fraza Team has requested to Vendor to be extremely careful to make sure on proper repairs of defective areas before the Application of the Finish Coating.

Re-Inspection: Repairs on defective areas by Re-Blasting and Re-Primer Coating, completed by Vendor in accordance with Referenced Project Documents. See Photo as Illustration of Referenced Issue as originally found.

Witness on Insulation/Fibering per ITP-50921-01 Rev 2 Item 16 and Customer Specification QRT70550 Rev 4.

Items Inspected: Transition Exhaust Item 42.

Findings: Observed at least two locations with Thru Splices-Lap (Splices of two consecutives fibering layers matching), Not Acceptable to Referenced Customer Specification Section 6.0.

Re-Inspection: Referenced Fibering Issues have been corrected in accordance with Requirements per Referenced Project Documents, accordingly the Fibering Process has gone ahead. See Photo as Illustration of Referenced Issue.

Findings: This Tank was delivered for Final Inspection by Fraza Team, however it was found not painted on underside of Roof, Not Acceptable to Customer Data Sheet EMR-TK-DTS-Y7502 Rev 02.

Visual Inspection on Hot Dip Galvanized Surfaces per ITP-104043-01 Rev 1 Item 13, Customer Spec 295-395435 Item 020 and ASTM A123/ASTM E376 Standards.

Item Inspected: Hot Dip Galvanized Structure Columns.

Findings: Observed a lot of areas on Galvanized Surfaces of Columns repaired by brush retouch since as explained from Vendor, the heat input from the Hot Dip Galvanizing produced a critical deviation on straightness of columns, which they tried to fix by heat application at an important number of locations, damaging locally the galvanizing, which was repaired subsequently by brush retouch. Fraza Team has reviewed this issue with Vendor Representatives and it has been confirmed and agreed, the quantity of locations repaired by brush retouch is Not Acceptable to ASTM Standards referenced above.

Re-Inspection: Per Customer Direction, all Referenced Columns have been Hot-Dip-Galvanized again in accordance with Standard ASTM A-123, as well all of them dimensionally inspected for straightness by Vendor QC Team and found acceptable to Project Requirements. See Photo as Illustration on referenced Issue.

Visual Inspection per ITP-104043-01 Rev 1 Item 3.

Item Inspected: MTT Filter Modules.

Findings: Flat Bars located within the MTT Filter Modules observed with their edges in direct contact, such condition does not permit a proper blasting and painting coverage on those areas. In opinión from Fraza Team, this is an Issue, Not contemplated by the Design from Vendor.

Re-Inspection: Vendor Engineering Team have confirmed on a correction plan as follows, one of the two flat bars in each couple of them is to be removed in order to leave the other one with its edge free to be properly surface prepared and painted in accordance with the Specified Paint System. Completion of referenced rework has been confirmed by visual inspection. See Photos as illustration on referenced Issue before and after correction.

Witness on Holiday Test on Internal Coating per Quality Plan ITP-104043-01 Rev 1, Item 17 and Customer Spec 295-NM-435 Item 030.

Item Inspected: LH End Plenum Module.

Findings: Six Paint Voids detected on internal stiffeners

Re-Inspection: All Voids detected have been repaired by paint re-touch and after permitted drying time in accordance with the Paint Manufacturer Data Sheet all of them have been Holiday Tested again with acceptable results. See Photo as Illustration on Holiday Test.

Findings and Conclusions: All Documents Acceptable to Project Requirements, Except Surface Preparation Reports which reflect on Specified Range for Anchorage Profile, 2.0 to 3.0 Mils per Customer Data Sheet NPR-TK-DTS-Y5002 Rev 01, Actual Profile Readings per Reports, 2.4 to 4.8 Mils, several of them higher than the maximum specified of 3.0 Mils, however, considered as no issue since the lowest Painting DFT applied is above 5.0 Mils, therefore all peaks per exceeded anchorage profile are fully paint covered.

Visual Inspection on Paint per Manufacturing Services Assurance & Integrity Plan CT-T13-001 Rev 2 Item 12 and Painting Specification-CHP-088-GT-207 Rev 0.

Items Inspected: Christmas Trees Serial Numbers CH-T-13-0295/296/297/298.

Findings: A lot of small circular valleys observed on painted surface with No exposure of the underlying coating nor substrate.

Re-Inspection: All surfaces observed with this paint defect have been repaired by sanding and re-top coating application with Acceptable Results. Looking for the root cause of referenced paint defects, Fraza Team has recommended vendor to conduct Compressed Air Blotter Tests in accordance with Standard ASTM D4285 to discard the presence of Oil and Moisture in the compressed air being used for blasting and painting. Re-Inspection: Vendor has applied later the specified Top Coating, found missing originally. See Photo on Equipment Top Coated.

Visual Inspection on Paint per ITP-1786-02 Rev 1 Item 12 and Documents as referenced below.

Item Inspected: Knock Out Pot-Tag HF1-RT886.

Findings: Paint system per Paint Code T330U from Specification 8HFQ-7603 Rev 3 with Primer Hi Temp and subsequent Top Coating 500VS had been completed, however a lot of days have passed after paint application and the applied coating has not cured since it has not hardened, showing a surface excesively soft easy to remove.

Item Inspected: Wet Steam Separator Tag Number FM-3743.

 Findings: Primer Coating being applied, Inorganic Zinc Primer Interzinc 22HS. Vendor does not use a Paint Pot with integrated Continuous Agitator, They just fill the mixed primer in the deposit of the Gun and apply the Primer Coating. Under these conditions with no continuous agitation the powder in suspension is settled on the bottom of the gun deposit, therefore an application very poor in Zinc Powder is done. Not Acceptable to Manufacturer Data Sheet of Inorganic Zinc Primer Interzinc 22HS and Customer Spec FM-T24993-01 Rev B Protective Coatings, Section 4.5-3, which requires for Continuous Agitation during Spray Application.

Re-Inspection: Per Customer Direction, Vendor has removed by blasting the applied Primer Coating and has started to use a Paint Pot with integrated continuous agitator for inorganic zinc application, Fraza Team has witnessed and confirmed the agitator works properly powered by the compressed air line from Vendor Painting Station.       Item Inspected: Hot Dip Galvanized Structure Columns.

      Findings: Observed a lot of areas on Galvanized Surfaces of Columns repaired by brush retouch since as explained from Vendor, the heat input from the Hot Dip Galvanizing produced a critical deviation on straightness of columns, which they tried to fix by heat application at an important number of locations, damaging locally the galvanizing, which was repaired subsequently by brush retouch. Fraza Team has reviewed this issue with Vendor Representatives and it has been confirmed and agreed, the quantity of locations repaired by brush retouch is Not Acceptable to ASTM Standards referenced above.

      Re-Inspection: Per Customer Direction, all Referenced Columns have been Hot-Dip-Galvanized again in accordance with Standard ASTM A-123, as well all of them dimensionally inspected for straightness by Vendor QC Team and found acceptable to Project Requirements.       Findings: Flat Bars located within the MTT Filter Modules observed with their edges in direct contact, such condition does not permit a proper blasting and painting coverage on those areas. In opinión from Fraza Team, this is an Issue, Not contemplated by the Design from Vendor.

      Re-Inspection: Vendor Engineering Team have confirmed on a correction plan as follows, one of the two flat bars in each couple of them is to be removed in order to leave the other one with its edge free to be properly surface prepared and painted in accordance with the Specified Paint System.       Item Inspected: Wet Steam Separator Tag Number Y-3742.

      Findings: Observed scale inside boltholes on Inlet/Outlet Flanges and at other areas, such scale was Not Removed by Blasting during Surface Preparation. Not Acceptable to Specified SSPC-SP 10 Near-White Metal Blast Cleaning.

      Re-inspection: Per Customer Direction, Vendor has re-blasted the equipment to remove completely any scale indications, in compliance with Specified SSPC-SP 10 Cleaning. See Photos on Referenced Issue before and after Re-Blasting.

      Item Inspected: Wet Steam Separator Tag Number FM-3743.

       Findings: Primer Coating being applied, Inorganic Zinc Primer Interzinc 22HS. Vendor does not use a Paint Pot with integrated Continuous Agitator, They just fill the mixed primer in the deposit of the Gun and apply the Primer Coating. Under these conditions with no continuous agitation the powder in suspension is settled on the bottom of the gun deposit, therefore an application very poor in Zinc Powder is done. Not Acceptable to Manufacturer Data Sheet of Inorganic Zinc Primer Interzinc 22HS and Customer Spec FM-T24993-01 Rev B Protective Coatings, Section 4.5-3, which requires for Continuous Agitation during Spray Application.

      Re-Inspection: Per Customer Direction, Vendor has removed by blasting the applied Primer Coating and has started to use a Paint Pot with integrated continuous agitator for inorganic zinc application, Fraza Team has witnessed and confirmed the agitator works properly powered by the compressed air line from Vendor Painting Station.       Item Inspected: Pump Tag RNP 35A Case and Cover Assembled together.

      Findings: Cover found installed/bolted to Case, as per vendor this done to protect the internal machined surfaces of the Pump Case from the Blasting action: The exposed CS Surfaces between the interface of Case and Cover will not receive the required blasting preparation and painting spray since the gap between both parts is extremely closed, therefore the blast media as well the paint spray will not reach properly these areas to ensure a complete blast and paint Coverage.

      Re-Inspection: Attending the observation from Fraza Team, Vendor has finally addressed to make sure a full coverage from blasting and painting on referenced áreas has been achieved. See photo below.

      Findings: Plan 21-Cooler Vessel observed with 6¨-WNRF and Blind Flanges with no surface preparation per specified cleaning  ISO-Sa-2.5 (SSPC SP10) since the applied painting is showing lack of adherence, getting off, leaving discovered the substrate no surface prepared.

      Re-Inspection: Vendor has completed on repairs by re-blasting and painting on such areas in accordance with requirements per Project Documents referenced above. Addressed by Vendor. See photos as illustration.

Specified Total DFT Range per Vendor Paint Spec above: 8.0 to 12.0 Mils: Actual DFT Measurements: 11.5, 13.8, 14.5, 16.3 and 15.7 Mils Total Average-14.3 mils. Out of Tolerance to Customer Specification-SERJ-I-003 Rev 2 and SSPC-PA-2 Standard.

      Re-Inspection: Vendor has completed on repair to bring final DFT within tolerances per Project Specifications. See Photos on referenced Issue.

      Items Inspected: Columns and Motor Support.

      Findings: Adhesion Test Results: Items with test spots showing Peeling(Flaking), Item 223702527 Middle Column, Item 223702530 Upper Column and Item 223702533 Motor Support, Not Acceptable to Section 7.3.2.7 of Vendor Procedure BV-934-01 Rev 01. Failed Results reflect a poor condition of adhesion of the paint film on tested areas, which can be caused by an irregular and inappropriate surface preparation or an irregular anchorage profile.

      Re-Inspection: Fraza team has recommended to Customer to blast mínimum to SSPC SP 6 Commercial Cleaning all ítems with rejected adhesión test in order to achieve a suitable and reliable condition of adhesion. Subsequently addressed by Customer. See Photos on Adhesion Test Spots.

      Item Inspected: Strainers Tag Numbers 12-HT-G-SYU-0056-A/B/C, 11-TH-SK-0139-A/B/C and 14-TW-IUN-087-A/B.

      Findings: Referenced Strainers were not painted in interior surface, Not acceptable to Specification 465732-T-U-038 Rev 1 Section 8.9.2 which specifies for ¨Internal Strainer Body and Head shall have a proven marine or epoxy lining to be applied in the Sellers Shop. Also, Spec 460715-T-U-036 Rev 0 Page 7 Section 2.7.3-b)-2) specifies too for Painting to be applied in Exterior and Interior Surfaces.

      Re-Inspection: Issue finally addressed/corrected by Fabricator in accordance with referenced Project Requirements. See Photo as Illustration on Referenced Issue.

      Findings: Referenced Strainers were painted in interior with a Epoxy Coating in accordance with internal Fabricator Standards, however this Applied Epoxy Coating in interior surface was not examined by a Continuity Test by a nondestructive detector as specified on Specification 465732-T-U-038 Rev 1 Section 7.13.2.

      Re-Inspection: Vendor has conducted a Continuity Test by a nondestructive detector with aceptable results as specified per Project Requirements. See Photo on Continuity Test as conducted.
