Visual and Dimensional Inspection per Inspection Attribute List # 11 and Drawing MTV-2650-DWG-01 in Status “No Revision”.
Item Inspected: Oil Water Separators.
Findings: Clips for Ladder Installation placed in Opposite Position to Specified Position per Drawing MTV-2650-DWG-01 Rev 2. This critical issue has been caused due to Vendor has worked with an old(obsolete) Revision of the Drawing.
Re-Inspection: Major Correction has been completed by Vendor to locate ladder clips in right position in accordance with current versión of Drawing as approved from customer. See Photo as illustration on original issue as found.
Visual and Dimensional Inspection per Inspection Attribute List # 17 and Drawing MWS-5432-DWG-01 in Status “No Revision”.
Item Inspected: Work Platform and Ladder.
Findings: Fabricated in accordance with an Old and Obsolete Version of Drawing MWS-5432-DWG-01. No Acceptable to Current/Approved Version of Drawing MWS-5432-DWG-01 Rev 3.
Re-Inspection: Work Platform and Ladder Re-fabricated in accordance with Current/Approved Version of Referenced Drawing.
Benefits to Our Customer: By using Fraza Inspection Team, Customer saved a lot of of time and money preventing excessive expenses per back charges for Reworks at field site produced per Unacceptable Issues from Control of Documents if Not Addressed in timely manner at Vendor Shop.

Visual Inspection on External Fiber Glassing(FRP) per Inspection Attribute List # 16 and Data Sheet TOB-JH-DS-U-IR-01 Rev 1.
Item Inspected: Circular Extension of Oil Water Separator.
Findings: External Fiber Glassing(FRP) of Circular Extension observed with Lack of Adherence between the Steel and the Fiber Glassing (FRP) Material, behind the Field Connection Flanges.
Re-Inspection: Repairs completed by Vendor achieving a proper adherence between the Steel and the Fiber Glassing (FRP) Material. See Photos on described Issue.

Visual Inspection on Welds per Inspection Attribute List # 10 and Drawing 7MU-CG-298-5-21 Revision 3.
Item Inspected: Oil Water Separator.
Findings: Underfill on Circumferential welded joints of main shell and welded joints of Rectangular Extension of Separator.
Re-Inspection: Repairs Completed by Vendor. See Photos as illustration on referenced issue.
Witness on Air Test per Inspection Attribute List # 13 and Data Sheet TOB-JH-DS-U-5L-01 Rev 2.
Item Inspected: Oil Water Separator.
Findings: Air Test at 6.0 psig on Separator, a leak detected on circumferential joint of separator shell.
Re-Inspection: Leak Repaired by Grinding and Welding by Vendor and Re-Air Tested at 6.0 psi, found with No Leaks and No pressure drop. Properly Addressed by Vendor. See Photos as illustration on referenced issue.
Review on Status of Engineering Documents per Inspection Attribute List # 02 and Customer Submittal FG-276-21 Dated October 2012.
Findings: Drawing MTV-2650-DWG-01 in Status “No Revision” observed with No Evidence on Customer Approval and being followed by Vendor to fabricate the Oil Water Separator, however Fraza Team has received from Customer the same Drawing in Status of Revision 2 Approved from Customer.
Re-Inspection: Per suggest from Fraza Team, Customer has provided to Vendor with a complete set of Updated Revisions of Drawings, preventing the use of any obsolete versión of drawings by Vendor.
Benefits to Our Customer: By using Fraza Inspection Team, Customer saved a lot of of time and money preventing excessive expenses per back charges for Reworks at field site per unacceptable issues on control of documents, welding and air tightness of separator vessel, if Not Corrected/Repaired in timely manner at Vendor Shop.
