Vendor Location: Fabricator of Heat Exchangers in Durango, Mexico.

Witness on Pneumatic Test per ITP WHM-1176-593-006 Rev 2, Activity Number 8.13 and GA Drawing 748W0342 Rev D.

Item Inspected-Oil Cooler Tag Nr WHM-2177, Pneumatically Tested on Tubes Side.

Findings: Test pressure was taken to MAWP pressure at 229 psi, at that pressure was detected a leak at a pipe nozzle connected with the shell. Leak produced by failures on expanded joints of Tubes to Tubesheet, transmitting pressure from tubes side to shell side.

Re-Inspection: Vendor has corrected the failures found on joints of Tubes to Tubesheet and Pneumatic Test conducted again in accordance with Project Requirements with Acceptable Results. See Photo as illustration of Leak originally found.

Review on CMTRs per I&TP MRU-9711-395-1054-004 Rev 2, Activity Number 3.11, GA Drawing 847E0253 Rev D and Material Requisition MR-9711-452-01 Rev 1:

Item Inspected Oil Cooler MKH-5421, Tag Number T9542.

Findings: During the Process of review of CMTRs, It was discovered that the tubes installed in referenced Heat Exchanger were wrong, Material Requisition MR-9711-452-01 Rev 1, section 9.1 specifies for  finned tubes and really plain tubes have been used by Vendor.

Re-Inspection: As per Customer Disposition on NCR-400-5697, Vendor has proceeded to remove the existing plain tubes and replaced with the right ones finned.

Benefits to Our Customer: By using Fraza Inspection Team, Customer saved a lot of of time and money preventing excessive expenses per back charges for Re-works at field site per unacceptable issues on Oil Cooler Air Tightness and Wrong Materials, if Not Corrected/Repaired in timely manner at Vendor Shop.
